Dear Parent,
On behalf of all the staff, we would like to welcome you and your children to our office. We enjoy treating children and are pleased that you have selected us to care for your dental needs. We want you to know that we are committed to providing you the highest quality of oral health care in the most gentle, efficient and caring manner possible and we look forward to making the visit pleasant for you and your children.
During your first visit, we will clean your child’s teeth and check for decay. This examination will include any necessary x-rays to help us make an accurate diagnosis of the condition of your child’s mouth, teeth and gums. In most instances, we will determine your child’s dental needs and then discuss the suggested treatment with you. If necessary, we will make follow-up appointments for future care.
Enclosed you will find a Health History Questionnaire. Please read and complete it and bring it with you to your first visit. Should you have dental insurance, please bring the completed and signed form with you. We would appreciate payment for this initial examination on your initial visit.
We strive to always be on time for you, except as emergency situations arise, and would appreciate the same courtesy. We understand that unexpected commitments can and do arise. Please notify our offices of any changes in schedule or cancellations at least 48 hours in advance to avoid a $25.00 missed appointment fee, and so that we may make our appointment times available to other children who are desperately in need of treatment.
Many of our parents ask how to prepare their child for their first dental visit. We suggest that you be very positive, stressing that a visit to the dentist will help your child keep a bright, attractive smile and that dental visits are a part of growing up. The enclosed information included in this brochure will give you some helpful hints.
